Why do I have to complete an intake form in order to get a massage?

It is extremely important to be forthcoming with your medical history when completing the intake form. There may contraindications that will prohibit the therapist from working on you. The therapist will always err on the side of caution. The client’s welfare is our primary concern. The State of Texas requires that forms be kept on file securely along with the therapist’s assessment should any problems arise.
